A recent study by the British health campaigning charity Action on Smoking and Health (ASH) found that the number of people in that country who reported trying electronic cigarettes more than doubled, from 9 percent in 2010 to 22 percent in 2012.

Almost all the new users were people who already had smoked cigarettes and 40 percent of them hoped switching would free them from the usual hazards of smoking.

Those hazards include an increased risk of dying from cancer, mostly due to inhaling the tar and other toxins released by tobacco when it is burned.

But are electronic cigarettes, or e-cigarettes, really safer than smoking?

Amanda Sandford, a research manager for ASH in London, says the only tobacco product the new devices contain is nicotine.

“Nicotine, compared to tobacco smoke, is relatively benign. It is the addictive component of tobacco, of course, and that is what keeps people coming back to smoking, but the harm [in smoking] comes largely from the inhaling of tobacco smoke,” Sandford says.

“Nicotine itself, once it is isolated and extracted from tobacco and just used in its pure state, is relatively harmless.”

E-cigarettes, which are inhalers, use a small battery to heat a dose of liquid nicotine into mist. The mist also contains –

And even a solvent for flavorings that are not very soluble on their own.

Sandford says it is still too early to know for certain the health effects of e-cigarettes because the products only arrived in Britain in quantity two to three years ago. Extensive medical testing still has to be done, leaving the door open to questions.

One question is whether people will smoke more e-cigarettes than they would conventional cigarettes because they believe they are safer, and what health risks the increased nicotine addiction might bring.

The BMA is worried that the more people start using e-cigarettes the more it will normalise something that looks like smoking. They have called for the ban on smoking in public places to be extended to e-cigarettes.

A decision on whether the regulation of electronic cigarettes should be tightened will be made in a few weeks.

Lynne Dawkins and her colleagues from the University of East London write in the journal Addiction that there are currently over 100 brands of e-cigarettes, and 3.5 million devices were sold in 2012.

Despite the devices’ growing popularity, the researchers say, little is known about who uses e-cigarettes and why.

For the new study, they created an Internet survey that was accessible from the websites of two e-cigarette manufacturers from September 2011 to May 2012. The survey took about 15 to 20 minutes to complete.Overall, 1,123 ex-smokers and 218 current smokers from 33 different countries took the survey. About 16 percent of participants were from the U.S. and another 77 percent were from Europe. Seventy percent were men.

About three quarters of respondents said they started using e-cigarettes as a “complete alternative to smoking,” and 22 percent said they started using the devices for “other reasons” – including stopping smoking (7 percent), for health reasons (6 percent) and to get around smoking restrictions (3 percent).

Some 86 percent said they had either not smoked cigarettes for several weeks or months since using the e-cigarette or that the amount they smoked had decreased dramatically.The researchers also found that the majority of people responding to the surveys felt their health had improved since using the devices.

“Most people reported great health benefits. Namely –

  • Their cough was reduced
  • Their breathing was improved,”

said Dawkins, who added that the benefits are most likely from people smoking fewer cigarettes and not an effect of the devices or vapors.

Still, Dawkins told Reuters Health that more research is needed on the long-term effects of e-cigarettes.

Siegel said there’s no question that e-cigarettes are safer than smoking, but there are concerns over some of the vapors’ ingredients – including propylene glycol, which irritates airways, and formaldehyde, which is known to raise lung and nasal cancer risk when it’s inhaled.

Boston University’s Siegel said the dangers are greatly exaggerated. While it’s true that e-cigarettes contain trace levels of nitrosamines, so do approved nicotine replacement therapy products, such as patches and gums. The amounts are very low and don’t appear to cause any health problems, he said. And unlike those products, e-cigarettes afford smokers who are hooked on the act of holding a cigarette, taking a drag and inhaling, a very similar experience.

“We know hundreds of thousands of people are dying from cigarettes, yet those are perfectly legal,” said Siegel, who does not accept financial compensation from any e-cigarette company. “If [Health Canada is] going to ban anything, why not ban tobacco cigarettes?”

The real potential of e-cigarettes is harm reduction, said Theo Moraes, a pediatric respirologist at Toronto’s Hospital for Sick Children.   • When I was smoking cigarettes, I would smoke maybe a half pack a day. But I’m not convinced I’m getting more nicotine per day puffing on e-cigarettes than I would if I were smoking cigarettes. 
  • This is because my e-cigarette inhaling habits vary wildly and I suspect the nicotine levels also vary from e-cig to e-cig and refill to refill. It is easy to suck on one of these things all day, like an adult pacifier. It’s also easy to take inconsistent drags, and each brand has a different draw. Also, the lower the battery the harder you have to draw on the e-cig to get a proper hit.
